Below is the Certified Family Peer Specialist (CFPS) online application. Please refer to the Candidate Handbook for additional questions about becoming a CFPS.

 

The following is required for FPS certification:

• Completion of the application below. Note: Per CFPS eligibility requirements, the application will require documentation of at least five hundred (500) hours of work (paid or volunteer) in the past five (5) years. Paid or volunteer activity done on behalf of your own immediate family cannot be counted in this section.

• Documentation of 88 contact hours of training related to the 5 national core competencies (within the past 5 years). Upload the required Training Documentation form linked below with your application. 

• Completion of the Supervision Verification Form linked below. A minimum of 20 hours of supervision is required.

• $350 which includes a non-refundable $50 application and $300 certification fee.

We encourage you to have your resume and training documentation form ready prior to beginning the application. Additionally, a copy of the CFPS Code of Ethics, which you will review and sign in the application, is provided below to download for your own records.
