2012 Application and Candidate Handbook

The 2012 Pilot CPSP Exam Application and Candidate Handbook are now available! If you submitted an application for the November testing, you DO NOT HAVE TO REAPPLY. If you have questions about certification, the application or the handbook, please contact us at certification@ffcmh.org or call 240-406-1472.

2012 Pilot CPSP Application

The 2012 Pilot CPSP application can be downloaded two ways. You can download the full application or you can download it in parts. If you download it in parts, please be sure to compile all of them again before you send it in to make sure you completed all sections.

Please also note that the application can be filled out directly in the PDF for your convenience. To fill out the application on your computer, just click in each section of the application. You will then be able to fill out that section. Unfortunately, you cannot save your application if you choose to fill it out online so allow enough time to fill out the application in full. Be sure to print a copy of the completed form before you close the page. Finally, you may either sign this with a electronic signature using Adobe Reader’s signature ability or you may print out your completed application and sign it manually.

To download the PDF, you’ll need the latest version of Adobe Reader which is free to download: http://get.adobe.com/reader/. Most computers should already have some version of Adobe Reader installed. If you have any issues opening the PDF, feel free to contact Corey Brown at cbrown@ffcmh.org.

You have two options to pay. You may use the form above to pay or you may use PayPal to pay. If you use PayPal, there will be a service charge of $2.00 for the application fee and $9.00 for the exam. PayPal charges us to process online payments so the extra charge is to cover the difference. We apologize for the inconvenience.
